The calculator uses the Stern formula, derived from the San Antonio Heart Study (a prospective cohort of nearly 3,000 adults): risk = 100 / (1 + e^(-(0.028 × age + 0.661 × sex + 0.079 × fasting glucose + 0.018 × systolic BP — 0.039 × HDL + 0.07 × BMI + 0.481 × family history — 13.415))). Each factor is weighted based on its statistical association with incident diabetes. This risk assessment is intended for adults aged 18 and older. The original study population was predominantly non-Hispanic white and Latino from Texas. While the formula performs well in these populations, caution is advised when generalizing to other ethnic groups. For example, in South Asian populations (India, Pakistan, Bangladesh), the Indian Diabetes Risk Score (IDRS) is a commonly used alternative that includes waist circumference and physical activity.

India: Diabetes affects over 77 million adults in India (ICMR 2023). The IDRS (Indian Diabetes Risk Score) developed by the Madras Diabetes Research Foundation is often used in Indian clinical settings. Our calculator uses the Stern formula which may underestimate risk for South Asian populations who have higher diabetes susceptibility at lower BMI thresholds. Fasting glucose units in India use mg/dL (same as mg/dL).
United States: The Stern formula was developed on a US population and is most accurate for non-Hispanic white and Latino adults. The CDC reports that 38 million Americans have diabetes (2023). For US Latin American users, an ethnicity coefficient of 0.412 is part of the full Stern model. Normal fasting glucose: 70-100 mg/dL.
